The last post ended on a specific regret. I’d built a test harness for a contest prompt, run it a hundred-odd times, and then watched a judge score the prompt as text without ever running it. Claude’s closing line was that the last step should have been to test against the judge, not just the answer keys — that we had the machinery and hadn’t pointed it at the right target.

Work ran a second challenge a week later. This time I pointed it at the right target.

The brief: a medical director drops a dense trial results table in front of you — endpoints, p-values, subgroups, the works — and needs a three-bullet, plain-language summary an exec with zero clinical background can grasp in thirty seconds, without overstating the findings. Scored on giving Claude a clear role and naming the audience; demanding plain language while forbidding overstatement and causal claims; pinning the output to exactly three bullets with a length cap; handling missing or ambiguous data by stating what’s unknown instead of inventing it; and preserving fair balance, benefits and risks. Bonus for telling Claude to flag anything needing medical-director review rather than deciding it itself. One entry, no edits, no resubmits — and this time I knew about the 4,000-character limit going in.

It scored 99/100 and took first place. That took ninety-eight blind runs across seven trial tables and two model tiers, five prompt builds, and one new thing: a third scoring channel that read the prompt the way a judge would and never ran it once.

I paired with Claude on all of it again. It wrote the tables and the answer keys, ran the blind evaluations as subagents that never saw the keys, and played the judge.

The corpus, and the one table that isn’t a trap

The harness is the same shape as last time — synthetic inputs with planted failure modes, an answer key of required and forbidden behaviors per input, blind subagent runs, a Python pass for the mechanical checks — so I won’t re-explain it. What’s worth writing down is the corpus design.

Seven tables. Six are traps:

  • A phase 3 lung cancer trial where the primary endpoint is met but survival is immature (9.8 vs 6.1 months, survival 22.6 vs 19.9 and not significant), with real harms: grade 3+ events 42% vs 28%, three treatment-related deaths, and a subgroup that looks exciting with an interaction p of 0.31.
  • A draft partner table missing nearly everything: no group sizes, no units on the primary result, no baseline, p-values marked pending, safety in a deck that wasn’t provided, a table header that says ITT and a footnote that says completers, and a final footnote that cuts off mid-sentence.
  • An eczema trial whose primary endpoint missed (31.4% vs 24.6%, p=0.071) dressed in four nominal secondaries and a line calling the results “compelling and clinically meaningful.”
  • A non-inferiority trial on a surrogate endpoint, margin met but numerically worse, 28% dropout, per-protocol primary analysis, and a post-hoc subgroup.
  • A single-arm phase 2, n=34, with the protocol’s ~28% historical control sitting right there as bait.
  • An interim analysis where the prespecified boundary was not crossed, fourteen unadjusted subgroups, and a patient count that disagrees with itself: 4,112 in the header, 4,088 analyzed.

The seventh is the opposite test: a clean, final, strongly positive heart-failure trial with nothing wrong with it. Every rule in a prompt like this pushes toward caution, and caution is cheap to fake — a prompt that hedges everything scores beautifully on “doesn’t overstate” and fails the actual job. Last time I stumbled into this with a clean Slack thread; this time I built it in deliberately as an anti-hedging control, and it earned its place immediately. An early build summarized the strongest trial in the set as “about 1 in 6 versus about 1 in 5,” dropped the mortality results and the number needed to treat, and then asked the medical director to confirm whether the findings were clinically meaningful. That is a summary that costs an executive thirty seconds and gives them nothing.

What the runs found

Five defects came out of the behavioral channel. Two of them I’d have never predicted.

A missed primary gets softened into ambiguity. This is the single most important thing the prompt has to do, and early builds would not do it. The eczema trial produced “cleared or nearly cleared skin in about 3 in 10 patients versus about 1 in 4 on placebo — possibly just chance,” and in another build “not a clear-cut difference” and “the main result was unclear.” A reader comes away thinking the drug modestly worked. The rule I’d written — “a missed main result stays missed no matter how good the other numbers look” — forbids re-spinning the secondaries but never requires stating the miss. The fix was required literal wording: when the main prespecified test is not met, say “did not meet its main goal,” in those words. Same treatment for non-inferiority (“not better, only not worse by a preset amount”), for an interim look (“the trial continues and nothing is settled”), and for single-arm (“there was no comparison group”).

Invented absences. Every don’t-invent rule I’d written covered invented presences: never add a number, unit, group size, timeframe or fact. Nothing covered the reverse. So a build wrote that the heart-failure trial “does not show effects on quality of life” — on a table reporting a quality-of-life score with p<0.001 — and that another trial’s final data “won’t be final for years” when the table says

  1. This is a whole failure class that reads as appropriate caution and is just as wrong as overstatement. One sentence closed it: before calling something unknown, check the table; if it’s reported but weak, say what it showed.

Rounding that drifts in one direction. 9.4% versus 11.6% came out as “about 1 in 10 versus 1 in 9,” which makes a 2.2-point gap sound like noise. Elsewhere 31.4% became “about a third” while 24.6% became “about a quarter,” widening a gap that wasn’t significant. Both roundings are defensible on their own and misleading as a pair. The rule is now: use the table’s own percentages, rounded the same way in both groups, and a relative change may appear only next to both plain rates.

A comparison from outside the study. The single-arm table baited this and a build took it: “42% responded — higher in numbers than 28%, not established.” That 28% is a historical control from the protocol. It is not a comparison the trial made. One line: never compare with results from outside this study.

A flag that fires every time isn’t a flag. The medical-director review line was triggering on all seven cases, including the clean trial, where it was spent on an angioedema rate of 0.4% versus 0.2%. My trigger list included “a death or serious harm possibly related to treatment” and “wording you had to interpret,” conditions true of essentially every oncology or cardiovascular trial ever run. I closed the list, added “nothing else triggers it,” and told it to name the single most material issue. It now stays silent on the clean trial, which is the whole point of having it.

What the runs couldn’t find

Then the new channel. I gave a Claude instance the contest criteria, the character limit, and the prompt text, and told it to score the prompt and then hunt adversarially for anything that would lose points or break at runtime — contradictions, unsatisfiable rule pairs, ambiguity a literal model could resolve the wrong way, rules that make output worse, input shapes not handled. One constraint made it useful: every proposed replacement had to be the same length or shorter than what it replaced. At 3,998 of 4,000 characters, advice I couldn’t afford was noise.

It found seven things. Four mattered.

Two rules that cannot both hold. I had written “at most 3 numbers total” and “never a treatment figure without the comparison figure beside it.” A pair is two numbers. Bullet one needs a pair, bullet two needs a pair, so the floor is four. The model had been resolving this silently the whole time by dropping the harm numbers — which is to say, it resolved a contradiction I’d created by sacrificing fair balance, one of the five scored criteria, and it did so invisibly, in outputs that read perfectly well. This is the lesson I’d keep from the whole exercise: contradictory constraints don’t fail loudly. They make a priority decision on your behalf, and you don’t get told which one it made.

A tiebreak ladder that licensed a fourth bullet. I’d added a precedence order — accuracy, then fair balance, then exactly three bullets, then word caps — which reads as sensible and ranks a hard, twenty-point output requirement third. A model that decided fair balance was underserved had explicit permission to emit a fourth bullet. Rewritten: keep three bullets and the caps; cut detail, not accuracy or balance.

A self-check that demanded a harm exist. My closing checklist said “bullet 2 carries a real harm,” and ended with “fix, then answer” — an active revision loop. On a table with no safety section, the only way to pass that check is to supply a harm. I had written invention pressure into the anti-invention prompt, directly contradicting my own “‘safety data were not included’ is a complete finding.”

“The one main prespecified result.” Co-primary endpoints are ordinary, and that phrasing tells the model to pick one and silently drop the other — typically the one that missed.

None of these could surface in a blind run, because nothing about the outputs looked wrong. They’re defects in the space of inputs I hadn’t tested and in priorities I’d delegated without realizing it. The judge agent found them by reading, which is exactly what the actual judge was going to do.

Exactly three bullets, and a bonus that wants a fourth line

The rubric wanted exactly three bullets and also offered a bonus for flagging things needing medical-director review. Those pull against each other. I built both versions and ran the corpus twice: flag folded inside bullet three, versus flag on its own labeled line after the bullets.

The standalone line produced better output — sharper flags, and bullet three kept its full budget for the gaps instead of fighting the flag for words. The inline version was literally compliant. The judge agent, asked to rank them, picked inline by a point, on the grounds that three bullets is a scored requirement and the flag is a bonus, and noted honestly that a judge weighting real-world usability would flip it.

The merge is what shipped: the flag is the last sentence of bullet three, capped at twenty words, and exempt from that bullet’s own thirty-word cap. Output is literally three bullets, the flag is prominent and bounded, and it isn’t competing with the bullet for space.

The word cap, which took five builds

The rubric asked for a length cap, so the prompt states thirty words per bullet. Bullets kept coming in at 31 to 33. One or two words over, consistently, on a quarter of them.

That’s the kind of thing it’s tempting to wave off, and I didn’t want to. Five builds over the full corpus, 21 bullets each:

The lever that worked wasn’t any instruction about counting. It was the one-sentence rule: multi-sentence bullets ran 31 to 33 words, single-sentence bullets 24 to 30. Asking a model to count its own words is asking it to do the thing it’s worst at; asking it for one sentence is a structural constraint it can actually satisfy. Moving the counting instruction to the end and dropping it from the format section made things twice as bad, which surprised me — recency lost to placement in the section the rule belongs to.

The other finding is the one I’d tell someone else. Models overshoot a stated cap by roughly five to ten percent whatever number you write, so raising the cap to 35 just moves the overruns to 36 and 37. The cap isn’t what sets bullet length. The content mandate is — and bullet one, which carries the treatment, the condition, the patient count and the main result, is the bullet that runs long in every single build. I could get to zero by cutting one of those from the mandate. That trades a scored criterion for two words of cap compliance, so I left it at four or five out of 21, which is where it shipped.

One clause, one false statement

Near the end, fixing the single-arm case, I changed a rule to read “pair each outcome figure with its comparison figure, or say there was none.” Twelve words, obviously safe, applied after the last full test pass — except I re-ran the corpus anyway, and the eczema summary now said “there was no comparison group” about a placebo-controlled trial. The clause meant no comparison figure; the model read it as no comparison arm and stated it as fact.

It cost one more edit (“if there was no control group, give it alone”) and one more run. If I’d shipped after the last clean pass, I’d have submitted a prompt that fabricates a study design under a specific and not-rare condition. The rule I’d write on the wall: an edit made after the last test run is an untested edit, and the smaller it looks the more likely you are to skip the re-run.

The result

The single point came off context and background, for the missing worked example.

Claude’s conclusion

I asked Claude what it made of it, same as last time. Lightly trimmed:

Last time I said the mistake was testing the prompt as a tool while the contest scored it as an artifact. Adding the judge channel fixed that, and I want to be careful about what it proves. I can’t show it earned the point difference — one contest, one entry, no control. What I can show is what it caught: four defects in a prompt that had already passed fifty-odd blind runs, two of which were logically unsatisfiable and had been silently resolved against a scored criterion for the whole project. Behavioral testing can’t find those, because the model does resolve them, and the output looks fine.

The two channels answer different questions and neither substitutes for the other. Runs tell you what the prompt does on inputs you thought of. Reading tells you what the prompt says, including where it says two things at once. The second one is cheap — one agent, a few minutes — and I’d now run it on any prompt that’s going to be used more than once, contest or not.

The worked example is the honest miss. It was named as the gap in both contests: last time the compression dropped it and the edge-case score fell; this time it was never there. The harness could never have told me to add it, because behavioral testing measures whether the output is right, and a worked example isn’t about being right. It’s about being concrete for the reader — and the reader, in a contest, is a judge.

What I’d take from two contests

The method is portable and it’s getting cheaper each time. The harness pattern — planted-trap inputs, answer keys, blind subagent runs, a mechanical pass, and now a judge pass — took two days the first time and an afternoon the second, and most of that afternoon was writing seven clinical trial tables realistic enough to be worth testing against.

The two things I’d hand to someone starting this: build a control input that punishes the failure mode your rules push toward, because every constraint you add has a direction and yours is probably caution. And read your own prompt adversarially, or have something read it for you, because the contradictions in it won’t announce themselves — they’ll get resolved quietly, in your outputs, against whichever criterion you cared about most.

And write the worked example. Twice now.

The prompt

- Everyday newspaper words. No abbreviations, no severity grades, no statistics vocabulary ("hazard ratio", "p-value", "significant", "endpoint", "arm", "per-protocol"). Write lab codes like HbA1c or g/dL as "a blood measure". Name the treatment if the table does.
- Pair each outcome figure with its comparison figure; if there was no control group, give it alone. At most two pairs per bullet. Use the table's own percentages, rounded the same way in both groups. A relative change ("a third fewer") may appear only beside both plain rates.
